PATIENT RIGHTS & RESPONSIBILITIES
Patient Rights
- To be involved and be part of planning your care; to notification of changes in the plan from your physician of choice; to be able to make infomed decisions for your plan of care; to be infomed about the outcomes, treatment, and services provided including outcomes that are unexpected.
- To be free from and to report neglect, abuse,or exploitation.
- To be free from restraint or seclusion used for convenience or retaliation by healthcare providers. Restraint or seclusion may be used to keep the patient safe from him/herself, protect others, and will be discontinued when safety is assured.
- To have your personal doctor and any family member you decide infomed of your hospital admission.
- To receive care in a safe setting.
- To express complaints, ethical concerns, concerns about the safety or quality of care provided, or grievances and to expect a response within a reasonable time. Concerns and grievances may be given verbally to: Northwest Medical Center, Springdale, 479-757-4000; Northwest Medical Center, Bentonville, 479-553-4100; Willow Creek Women's Hospital, 479-684-3054. Written notifications of concerns should be addressed to "Administrator, " care of the facility in which the patient was hospitalized.
- To be informed on how to contact regulatory and accrediting agencies for concerns. - To have your medical records made accessible and have information explained, except when restricted by law.

To be informed about research procedures or treatment that may be considered in your care. To discuss and refuse the procedure if desired.

To examine and receive any explanation of your bill regardless of source of payment.

Patient Responsibilities
The effectiveness of health care is enhanced by the collaboration and cooperation of patients, their families or representatives in:

Providing information about your health, including your past illnesses, hospital stays, and use of medications, both prescription and over the counter.

Asking questions when you do not understand information or instructions, and informing your doctor when you cannot follow through with your treatment.

Expressing complaints or any issue of concern in a timely manner in order to enhance the quality of the care you receive.

Being considerate of the needs of other patients, the staff and the hospital.

Providing information for insurance and working with the hospital to arrange payment, when needed.

Recognizing the effect of your lifestyle on your personal health.
