Northwest Health System's Communities Home Health has provided skilled healthcare for homebound patients since 1980. As one of the largest home health providers in the Northwest Arkansas region, Communities Home Health serves patients who live in Washington, Benton, Madison and Carroll counties.

When you have an illness, injury, or debilitating condition, Communities Home Health is here for you. Our staff of experienced nurses, therapists, and aides can assist you with various treatments, therapy, and personal care on an intermittent basis.

How Do I Know if I Qualify for Home Health?

Our friendly staff is happy to discuss your needs and develop a plan to meet those needs.   Physician’s referral required. Some cases that may require Home Health are:  

  • Recent stay in a hospital or rehabilitation unit requiring nursing or therapy follow-up
  • condition that has worsened, requiring medication changes and monitoring
  • A new condition such as diabetes or Congestive Heart Failure
  • Post Surgical or other wounds that require assessment, and dressing changes. Our nurses are certified in wound-vac therapy, a new technique in wound healing.
  • Care and teaching for a new or troublesome Ostomy
  • Catheter care, education, and maintenance
  • Intravenous therapy (such as antibiotics or fluid replacement therapy)

How Will I Pay for Home Health Services?
Medicare, Medicaid, and many private Insurances cover Home Health Services. Patients are accepted without regard for race, creed, sex, age, nationality, ancestry, or handicaps and our staff will work with you to determine the method of payment that you qualify for before services are provided.
